1,451 AI-based medical devices have received FDA clearance in the U.S. In Germany, the first hospital chains are already deploying AI diagnostics in routine clinical practice. Yet in most executive boardrooms, AI in healthcare remains an IT topic. That is a strategic mistake – because when an AI system misses a diagnosis, it won’t be the CIO standing before a judge, but the executive management team.
Most hospital executives treat AI diagnostics as a procurement decision: Radiology needs a new tool; IT evaluates options; procurement negotiates contracts. That’s dangerously reductive. AI in diagnostics is a clinical decision-support tool – one that influences treatment pathways, raises liability questions, and triggers regulatory obligations. It belongs on the strategic agenda – not in the IT steering committee.
The reason is straightforward: If an AI system misses a stroke in CT analysis and the patient suffers harm, the issue isn’t a software bug. It’s a treatment decision based on an algorithmic recommendation. The physician’s duty of care remains unchanged – but executive leadership bears organizational responsibility. Did the board ensure the AI was clinically validated? Were clinicians trained? Are fallback processes defined? If not, things get uncomfortable – fast.
Starting August 2027, the EU AI Act’s high-risk requirements will apply to medical AI systems. While the MDR governs product safety, the AI Act adds a distinct layer: AI-specific risk management, strict requirements for training-data quality, technical documentation exceeding MDR standards, and mandatory systems for human oversight.
For hospital executives, this translates concretely: Every AI system used in diagnostics or treatment planning must be embedded within a formal governance framework. That includes selection (Which AI – and on what evidence base?), integration (How is it embedded into clinical workflows?), monitoring (Who oversees its real-world performance?), and fallback (What happens if the system fails?).
The Vara-PRAIM study shows how it’s done right: 463,094 mammography screenings, published in Nature Medicine, with a 17.6% increase in breast cancer detection. This isn’t a pilot – it’s top-tier clinical evidence. Yet even such rigorous validation doesn’t relieve executives of ultimate accountability for deployment.

1. Who decides on deployment – and who oversees it? AI diagnostics requires a clearly designated C-level owner – not just the CIO or the Medical Director alone, but a board member who grasps both clinical and regulatory dimensions. In hospitals successfully deploying AI, you’ll find either a dedicated AI Board or a Chief Medical Information Officer bridging both worlds.
2. On what evidence basis was the system selected? 24% of FDA-cleared AI devices lack clinical validation studies. For executives procuring such tools, the critical question isn’t “Is it approved?” but rather “Has it been validated on a comparable patient population?” A CE marking or FDA clearance is necessary – but insufficient. Due diligence in product selection is an explicit executive responsibility.
3. What happens when the system fails? Every AI implementation requires a documented, tested fallback process. If Aidoc can’t prioritize findings due to system failure, workflows must seamlessly revert to manual screening. That sounds trivial – but operationally, it’s demanding. And it must be rehearsed before a crisis – not during one.
A fair objection: If every AI deployment demands a full governance framework, doesn’t that stifle adoption? The answer is: Yes – partially. But the alternative – a hospital executive introducing AI diagnostics without structured accountability – is worse. Not because of regulation, but because of liability. The first court case where a judge asks the executive team, “What governance did you have for this AI system?” will reshape the entire sector. Better to be prepared.
Asklepios demonstrates with its Aidoc rollout across 25+ hospitals that governance and speed need not be mutually exclusive. KHZG funding covers the technology; the hospital operator ensures organizational readiness. The model works precisely because it’s driven from C-level – not by the IT department.
The MDR governs manufacturer product liability. The treating physician’s duty of care remains fully intact. Executive leadership bears organizational responsibility: ensuring the system is validated, staff are trained, and fallback processes are defined. Final legal clarification through case law is still pending.
The full high-risk requirements of the EU AI Act enter into force on 2 August 2027. Medical AI systems falling under the MDR or IVDR are automatically classified as high-risk.
There is no national registry. Asklepios deploys Aidoc across more than 25 hospitals. The Vara-PRAIM study involved 463,094 screenings across German breast cancer centers. The G-BA (Federal Joint Committee) is promoting routine AI use in radiology via its xR.AI initiative. Germany remains far from nationwide rollout.
Framework-specific costs (process definition, documentation, training) typically range between €50,000 and €150,000. AI systems themselves are often funded via KHZG grants. Ongoing monitoring and compliance costs depend on the number of deployed systems.
